Completing everything ahead of time allows us to prepare for your visit and means less paperwork when you arrive at the office.
What Should I Bring to My First Appointment?
- If you’re unable to complete your information through our secure online forms before your visit, simply bring a photo ID and your dental insurance card, if applicable. Our team will be happy to help you with anything else we need when you arrive.
Transferring From Another Dentist?
- If you’re changing dentists, recent dental records and X-rays can provide helpful information about your previous treatment and dental history.
